OSHC Enrolment Form

An enrolment form must be filled out before your child attends our program. We recommend completing an enrolment form regardless if you plan to access the service. This way in an emergency (say if you are held up in traffic) your child can access out of school hours care.

 Please note that you will also need to provide the following supporting documents:

> Statement of immunisation status

> Any allergy or medical management plans (if applicable)

> Any court orders relating to the child (if applicable)
